Solace hospice aims to re-set the standard for hospice care IN OUR community. As an independent, locally owned agency, Solace provides high quality, compassionate, individualized, medical, emotional and spiritual support for those living with an end stage illness in Northeast Indiana.

What is included in the hospice Benefit?

  • Our team is locally owned with a combined 25 + years of hospice and healthcare experience, ensuring we have the knowledge and skill to provide the highest quality of care. Our patients have the option to receive care from a registered nurse, CNA, chaplain, and medical social worker and hospice physician. We are there to support your loved one physically, emotionally and spiritually.

  • Our hospice team is readily available and organized to meet the needs of the patient and family system 24/7. We provide care in assisted living facilities, skilled nursing homes, and in the comfort of one’s home.

  • As hospice providers, we care not only for the patient, but family members and loved ones -throughout the entire process, as well. Our spiritual care team exists to support these family members with processing grief, identifying local bereavement resources, and offering memorial services after the passing of a loved one.

  • Medicare covers the cost of hospice benefits often including regular visits, supplies, and durable medical equipment related to the terminal condition. If one does not qualify for Medicare, some private insurance plans cover hospice and palliative services. hospice Levels of Care

Routine Home Care

This is the most common type of hospice service we provide. Routine care involves professional services to a patient wherever they call home.

INPATIENT CARE

Short-term inpatient care for complicated symptom management at hospice-approved facilities as determined by the hospice medical team ON an As needed basis.

CONTINUOUS Care

Short-term in-home care provided when medically necessary for acute symptom management on an as needed basis.

Respite Care

respite care allows up to five days of respite services for caregiver relief on an occasional basis at hospice-approved facilities.
